D2 vs. D3: The Two Forms You Actually Take
The vitamin D in supplements and fortified food comes in one of two forms: D2 (ergocalciferol), derived from plant and fungal sources, or D3 (cholecalciferol), derived from animal sources and also the form your skin naturally produces from sunlight exposure. Research generally suggests D3 raises blood vitamin D levels somewhat more effectively than D2, which is part of why D3 has become the more commonly recommended supplement form, though both are real, legitimate sources of the vitamin.
Calcidiol: What Your Blood Test Actually Measures
Once you consume or produce D2 or D3, your liver converts it into calcidiol, also called 25-hydroxyvitamin D. This is the specific form doctors measure when they order a vitamin D blood test, not the D2 or D3 you actually took. Calcidiol serves as the storage and circulating form of vitamin D, and its blood concentration is what's used to assess whether someone has sufficient, insufficient, or deficient vitamin D status.
Calcitriol: The Active Hormone Form
The final conversion happens in the kidneys, where calcidiol becomes calcitriol, also called 1,25-dihydroxyvitamin D, the active hormonal form that actually does the work of regulating calcium absorption and supporting bone metabolism. Calcitriol isn't something you'd typically take as a general supplement, it's tightly regulated by the body and is used medically only in specific circumstances under a doctor's supervision.
